Sound familiar?
You went into healthcare for patients.
HR keeps pulling you out of the exam room.
Independent practices compete with health systems for the same people, under rules most businesses never see. Here's what practice owners and administrators tell us most, and where the right PEO fits.
“The hospital down the street just poached our best MA.”
Health systems recruit with big-group benefits and retirement plans. A PEO gives an independent practice access to large-group health, dental, vision and 401(k), so you can compete for MAs, RNs and front-desk staff without a health system's budget.
“We're short-staffed, so everyone is on overtime.”
Extended clinic hours, call coverage, shift differentials and PRN staff make payroll complicated, and getting the regular rate wrong on overtime turns into wage claims. A PEO runs payroll that handles differentials, on-call pay and multiple rates correctly.
“Our front desk turns over every few months.”
Every departure means recruiting, onboarding, HIPAA training and system access all over again. A PEO's HR platform standardizes onboarding and training records, so new hires get productive faster and nothing gets missed.
“Needlesticks, patient lifts, and now a work comp claim.”
Clinical work carries real injury risk, and home health adds driving and patient handling. The right PEO brings safety programs and claims management, and many offer pay-as-you-go workers' comp tied to actual payroll.
“Are our providers W-2 or 1099?”
Physician, NP, PA and therapist arrangements vary widely, and getting classification wrong creates tax and benefits exposure. A PEO helps you set up provider compensation, classification and benefits eligibility the right way.
“Three locations, three ways of doing everything.”
Growth through new locations or acquisitions leaves you with different handbooks, PTO policies and pay practices. A PEO consolidates payroll, policies and HR onto one platform across every location.
Why practices use a PEO
Think of a PEO as your RCM partner
for your people.
You'd never have your physicians work the claims queue. Most practices hand revenue cycle to specialists so the team can focus on patients. A PEO (Professional Employer Organization) does the same for HR: payroll, benefits, workers' comp and compliance, run by specialists so your team can focus on care.

Get a second opinion
before you commit to a PEO.
You'd never sign off on a major treatment plan without a second opinion. PEO decisions deserve the same: benefit networks, work comp programs, pricing and contract terms differ widely. PEO Marketplace does the discovery, evaluation and onboarding work, so you land on the best-fit PEO without becoming a PEO expert.

What it looks like
A 60-person practice with three locations
A common picture: an independent multi-specialty practice with 60 employees across three locations, MAs and front-desk staff turning over, a small-group health plan and payroll juggling shift differentials and PRN staff. Here's how the administrator's week changes once the right PEO is in place. More real outcomes in our case studies.

Healthcare FAQ
PEO for healthcare:
what practice leaders ask first.
Reviewed by PEO Marketplace, a licensed Florida insurance agency (#L129491). Last updated October 2026.
What is the best PEO for healthcare practices?
There isn't one best PEO for every practice. The right fit depends on your practice type, clinical roles, states, locations, benefit networks and workers' comp needs. We screen 40+ vetted PEOs on those factors and compare the best fits side by side. Run a lab or research team? See PEOs for life sciences.
Why do medical practices use a PEO?
To compete for staff and get HR off the administrator's desk. A PEO gives a practice large-group benefits that help recruit and keep MAs, nurses and front-desk staff, plus payroll, workers' comp and compliance support from specialists. It's like outsourcing revenue cycle: specialists handle it so your team can focus on patients.
Which PEOs have the best dental and vision benefits?
It depends on the carriers and networks available where your staff live. Two PEOs can offer the same carrier with very different plan designs and costs. We compare networks, plan options and employee costs side by side. See how plans stack up in our benefits benchmark.
Does a PEO handle HIPAA compliance?
Not for your patient data. HIPAA privacy and security for patient information stays with your practice. A PEO can support the workforce side: training acknowledgments, policies in your handbook and secure handling of employee records. Ask each PEO exactly what its platform tracks.
Can a PEO run payroll with shift differentials and on-call pay?
Yes, most can. Differentials, on-call and multiple pay rates also affect overtime, because they're included in the regular rate of pay. Confirm how each PEO calculates the regular rate and handles state rules before you switch, especially if you run extended hours or PRN staff.
Should our physicians and providers be on the PEO?
Employed physicians, NPs and PAs can usually be on the PEO, with payroll and benefits like other staff. True independent contractors stay off. If you pay productivity bonuses, such as wRVU-based comp, ask each PEO how it handles them in payroll.
Can a PEO cover workers' comp for home health aides?
Often, but not always. Home health and caregiving roles carry higher work comp risk because of driving and patient handling, and some PEOs limit them. We show you up front which PEOs write work comp for your roles and states.
How much does a PEO cost for a medical practice?
Expect an admin fee per employee or a percentage of payroll, plus benefits and workers' comp for your roles. Because each PEO bundles these differently, compare all-in cost per employee side by side. For a quick estimate, try our PEO cost calculator.
Will a PEO handle our malpractice insurance?
No. Medical malpractice (professional liability) coverage stays with your carrier. A PEO covers employment-related needs like workers' comp, and many offer employment practices liability insurance (EPLI) for claims from employees. For more on choosing, see our guide to the best PEOs for small business.
Is PEO Marketplace free for healthcare practices?
Yes. We work like a recruiter: our fee is paid by the PEO you ultimately choose. You get our team's discovery, evaluation and onboarding support at no cost, with no obligation to choose any PEO we show you. The platform itself is free for the first 50 businesses during the beta, with paid plans announced at launch.
